You are requesting an appointment for your dog. Please know that the following 4 vaccines must be received by our office before any appointments will be booked. Upon submitting this form you will then need to have the following 4 vaccines sent to us and once we receive them we will reach out to schedule an appointment:
We will need the following 4 current vaccinations:
- Rabies
- Distemper
- Bordetella
- Canine Influenza
These 4 vaccines can be email to us at: info@beau-tyes.com or faxed to us at 866-226-8772. Again, no appointments will be booked until these 4 current vaccines are received in our office first. Thank you.

Cancellation Policy: Because we book by appointment only, should you need to cancel your appointment for any reason, we require that clients call to cancel at least 24 hours in advance of your pets scheduled appointment date. Otherwise, a $50.00 rebooking fee charge will be applied (per dog).
